I am writing to request a home visit for Ms. Harwood. She is diagnosed with right partial rotator cuff tear. She requires care and assistance from your facility.

I am writing to request home care and assistance for the above-mentioned patient. She is recovering from right partial rotator cuff tear and is being discharged today.

Ms.harwood presented to our hospital with pain and weakness in the right shoulder, especially when lifting the arm overhead. X-ray and MRI confirmed she has right partial rotator cuff tear. Therefore surgeon suggested surgery, but she prefers to try the nonsurgical treatment. Hence commenced with Ibuprofen ( dosage), cortisone (Dose), and daily physiotherapy. 



Ms.Harwood is a known case of type 2 diabetic Mellitus and is on regular medication metformin 500 mg morning. Its worth to mention, she is a widow, lives alone, and has no children. 


After discharge, It is important to monitor her blood glucose level every 4 hour as it can be elevated due to the side effects of the cortisone injection. It would be beneficial for Ms. Harwood if you can assist her in activities of daily living such as shower and house works/ house chores.May I remind you, her orthopedic review scheduled on 21/03/2011 ( It would be highly appreciated if you can help her to attend an orthopedic review scheduled on Date XXX )
